 es La Troponina I Cardiaca: Marcador Bioquímico de Elección del Daño Miocárdico

Resumen

En la actualidad, la troponina I cardiaca (TnIc) es considerada por algunos autores el marcador de oro para el diagnóstico del infarto agudo del miocardio (IAM). Su utilidad se resume en la alta sensibilidad y especificidad de los ensayos para su detección, lo que permite la confirmación y exclusión del IAM, la valoración del riesgo de los pacientes con angina de pecho inestable y diagnóstico de microinfartos, el monitoreo de los resultados de la terapia trombolítica y el diagnóstico del daño miocárdico perioperatorio. En este artículo se describen las características estructurales del complejo de troponinas, principal forma en que se libera la TnIc en el torrente sanguíneo. La cardioespecificidad de la TnIc es su ventaja fundamental sobre los marcadores de inflamación, los marcadores de trombosis y otros marcadores de necrosis cardiaca en el diagnóstico del daño miocárdico. Existen diferentes estuches comerciales para el análisis cuantitativo y cualitativo de la TnIc en el diagnóstico del IAM, los cuales difieren mucho en cuanto a sus límites de detección y al valor de corte. Existen dos factores fundamentales que pueden influir en esa variación: la utilización de anticuerpos monoclonales o policlonales diferentes en los ensayos y la ausencia de un patrón universal confiable de TnIc. Se puede considerar que la TnIc es actualmente el marcador bioquímico de elección para el diagnóstico del daño miocárdico, debido a sus ventajas sobre los demás marcadores que se utilizan con este fin.

Abstract

Nowadays, cardiac troponin I (TnIc) is considered by some authors as the gold standard for acute myocardial infarction (AMI) diagnosis. The high sensitivity and specificity of detection are the main characteristics of TnIc that allow confirmation and exclusion of AMI, risk assessment of patients with unstable angina pectoris, and diagnosis of microinfarcts, monitoring of thrombolytic therapy outcome and diagnosis of perioperative myocardial damage. In this article, the structural characteristics of the troponin complex are described, which is the main form TnIc is found in the blood stream. The principal advantage of TnIc over inflammation, thrombosis and cardiac necrosis markers is its cardiospecificity. There are several quantitative and qualitative assays for TnIc in the market, but they have different detection limits and cut-off values for AMI. There are two main reasons for this variation: the use of different monoclonal or policlonal antibodies in the assays, and the absence of a reliable universal standard for TnIc. It may be considered that TnIc is the gold biochemical standard for myocardial damage because of its advantages over the other markers used.
