Resumen

Desde que se propuso al virus de la inmunodeficiencia humana (VIH) como causante del síndrome de la inmunodeficiencia adquirida (SIDA), se han realizado múltiples investigaciones para establecer una relación causa-efecto entre estas dos entidades. No obstante, como también ocurre para otras afecciones virales, aún no se ha logrado demostrar claramente la vigencia de los postulados de Koch para el VIH. Aunque la mayoría de los investigadores coinciden con que el VIH es el causante del SIDA, no faltan quienes argumentan la ausencia de una relación entre estas dos entidades. El SIDA se caracteriza, en la mayoría de las personas infectadas, por un deterioro del sistema inmunológico del paciente que conduce a la aparición de afecciones infecciosas y no infecciosas que llevan finalmente a la muerte. Aprovechando las condiciones excepcionales que brinda la epidemiología y el control de la infección por VIH en Cuba, se realizó un estudio sobre la incidencia del sarcoma de Kaposi (SK), tuberculosis (TB) y neuropatía epidémica (NE) en individuos infectados (VIH+) (N = 1 303) y no infectados (VIH-) (N = 11 004 563) por VIH entre los años 1986 y 1996. Se calculó la razón odd para comparar la incidencia de afectados sobre los no afectados con estas entidades, en los individuos infectados y no infectados con el VIH. Encontramos que para aquellas afecciones incluidas en el SIDA había una mayor incidencia en los individuos infectados por VIH (infecciosa: TB, odd > 22; no infecciosa: SK, odd > 3 100). Para el caso de la NE, no incluida dentro del SIDA, se encontró mayor incidencia en individuos no infectados por el VIH (odd = 0,49). Estos resultados aportan nuevas evidencias acerca de la asociación entre la infección por VIH y las afecciones infecciosas y no infecciosas que acompañan habitualmente al SIDA.

Abstract

Since the human immunodeficiency virus (HIV) was proposed as the cause of the acquired immune deficiency syndrome (AIDS), many investigations have been conducted to establish the relationship between these two entities. Nevertheless, as also described for other viral diseases, the validity of the Koch's postulates has not been established for the HIV. Although most of the researchers consider that the HIV is the cause of AIDS, few argue that no relationship exists between these two entities. AIDS is characterized in most of the infected people by a debilitation of the immune system, followed by infectious and non-infectious diseases which finally result in patients death. Taking advantage of the exceptionally good conditions found in Cuba for the epidemiology and control of the HIV infection, we studied the incidence of tuberculosis (TB), Kaposi's sarcoma (KS) and epidemic neuropathy (EN) in HIV- (N = 11 004 563) and HIV+ (N = 1 303) individuals between 1986 and 1996. The odd ratio was calculated to compare the incidence of these diseases in HIV infected and non-infected people. The results indicated that for the diseases included in the AIDS (infectious: TB, odd > 22 and non-infectious: KS, odd > 3100), the incidence was higher among HIV infected individuals. For the NE, not included in the AIDS, the incidence in HIV non-infected individuals was higher than in HIV+ (odd = 0.49). These results added new evidences showing that for our study conditions, a cause-effect relationship exists between the HIV infection and the AIDS infectious (TB) and non-infectious (KS) diseases.
