Resumen

La fibromialgia es un síndrome que se caracteriza por dolor muscular generalizado y la presencia de 11 de 18 puntos sensibles, que están anatómicamente definidos. Es más frecuente en mujeres adultas, pero también se ha encontrado en niños. No se conoce la causa exacta del síndrome de fibromialgia, pero se han descrito alteraciones bioquímicas (baja serotonina, alta sustancia P, desequilibrio oxidativo), metabólicas (alteraciones en el eje hipotálamo-hipófisis-adrenal), neuroinmunológicas (sensibilización central), musculares (bajo colágeno, aumento en la fragmentación del ADN) y genéticas (predisposición familiar). Está relacionado con otras condiciones médicas como el síndrome de dolor miofascial, colon irritable, depresión, ansiedad, cefalea tensional, entre otras. El diagnóstico se basa en el cuadro clínico. El tratamiento consiste en educación, ejercicio aeróbico, terapia cognoscitiva-conductual y medicación con antidepresivos tricíclicos y analgésicos.

Abstract

The fibromyalgia is a syndrome characterized by widespread muscle pain and the presence of 11 from 18 tender points which are anatomically defined. It affects more women than men, but it can be seen in children. It doesn't know the exact origin of the fibromyalgia but the studies have showed biochemical (decreased serotonin, increased substance P, oxidative disbalance), metabolic (dysregulation of the hypothalamus-pituitary-adrenal axis), neuroimmunological (central sensitization), muscle (decreased collagen, increased DNA fragmentation) and genetic (familiar predisposition) alterations. It's related with other medical conditions like myofascial syndrome, irritable bowel, depression, anxiety, tensional headache, etc. The diagnosis is made with symptoms and signs. The treatment consists of education, aerobic exercise, cognitive-behavioral therapy and medications (tricyclic antidepressives, analgesics).
