 en Early frontal dysfunction in patients with High Blood Pressure

Abstract

According to former results on patients with High Blood Pressure (HBP), it was decided to use the Variable Resolution Electric Tomography Analysis (VARETA) to depict the cerebral areas in which the patient´s slow EEG abnormalities originated. 84 patients in two groups: G1 (24 patients, systolic blood pressure (SP)=140-159; diastolic (DP) =90-99), G2 (60 patients, SP ≥160; DP ≥100) and 35 control subjects were studied with VARETA measures derived from their digital EEGs. The generators for the slow activity were mainly localized on frontal regions with lateralization to the left hemisphere and mostly on medial frontal gyrus (MFG). These results are probably explained by the characteristics of the cerebral blood flow in the carotid system, mostly in the left side, as it originates directly from the aortic arch. MFG is a distal interterritorial area more sensitive to the hypoperfusion caused by the stress of hypertensive patients’ blood vessels.

Resumen

Por resultados anteriores en hipertensos sin complicaciones clínicas decidimos hacer un análisis mediante la Tomografía Eléctrica de Resolución Variable (VARETA) para determinar las áreas donde se originaban las alteraciones lentas obtenidas en el Electroencefalogama digital. 84 pacientes integraron dos grupos de acuerdo con su presión arterial sistólica (PAS) y diastólica (PAD): G1 (PAS=140-159; PAD=90-99), y G2 (PAS≥160; PAD≥100) vs controles sanos. A partir de los EEGd de los pacientes se obtuvieron los espectros de frecuencia y el VARETA de cada paciente para localizar los generadores de las frecuencias lentas. Los generadores se localizaron predominantemente en las regiones frontales con lateralización izquierda y principalmente en la circunvolución frontal medial (CFM). Esto depende de las características de la circulación cerebral y el mayor stress de pared que tiene el sistema carotídeo del lado izquierdo por su origen directo del cayado aórtico. La CFM es un área interterritorial más sensible a la hipoperfusión.
