Resumen

El lipoma intraóseo, extremadamente infrecuente, constitutye el 0,1% de todos los tumores óseos primarios. Su patogénesis ha sido considerablemente debatida en la literatura, pero no hay duda de que esta lesión representa una entidad distinta, con hallazgos radiolgicos confusos. Laó apariencia radiologica del lipoma intraoseo, depende de la composicion histologica de la lesión. Es fácilmente diferenciado de otras lesiones óseas primarias, en imágenes de Tomografia computarizada o Resonancia magnética, ya que ambas modalidades tienen la habilidad de documentar, el tejido adiposo en el interior de la lesion. Se recomienda la observación expectante de la lesión, reservando la cirugia solamente para las lesiones muy sintomáticas o con fracturas recientes o riesgo de fractura. Se presenta el caso de un paciente masculino de 50 años de edad quien posterior a una caida de su propia altura, presenta dolor de moderada intensidad, aumento de volumen y limitación funcional en el tobillo derecho; se realiza radiologia simple del tobillo observándose como hallazgo incidental, una lesión ocupante del espacio intracalcáneo. Se caracteriza la lesión por medio de la tomografia y la resonancia magnética. Posterior a la confirmacion del diagnóstico, la conducta es expectante, en vista de la buena evolución del paciente y de estar asintomático.

Abstract

The extremely infrequent intraosseous lipoma, comprises 0.1% of all primary bone tumors. Its pathogenesis has been considerably disputed in literature, but there is no doubt that this lesion represents a separate entity, with confusing radiological findings. Radiological appearance of intraosseous lipoma depends on the lesion histological composition. At Computed Tomography or Magnetic Resonance imaging, it is easily distinguished from other primary bone lesions, since both modalities have the ability to document the adipose tissue inside the lesion. The expectant observation of the lesion is recommended, reserving surgery only for very symptomatic lesions or patients with recent fractures or fracture risk. The case of a 50 years old masculine patient is presented, who following a fall from his own height, presents with pain of moderate intensity, swelling and functional limitation in the right ankle; plane radiology of the ankle is performed, observing as incidental finding an occupant lesion of the intracalcaneal space. This lesion is characterized through tomography and magnetic resonance. Subsequently to diagnostic confirmation, the behavior is expectant, in view of the patient's good evolution and of being asymptomatic.
