Paper received: August 2002 Code Number: is02015 A 55-year-old female patient presented with history of a swelling in the neck for 2 years. It had increased in size and was associated with severe pain for the past week. The patient did not give any history suggestive of hypo or hyperthyroidism. She had consulted a local doctor 2 weeks ago and had been advised fine needle aspiration cytology (FNAC) of the swelling. Seven days following the FNAC, which had revealed nodular colloid goitre, the swelling had increased in size. An ultrasonographic examination of the neck revealed localized fluid collection in the left lobe of the thyroid with the possibility of a haematoma or an abscess. Her total leukocyte and neutrophil counts were raised. Thyroid function tests were normal. Aspiration of left lobe done under aseptic precautions by a 5cc syringe, confirmed the diagnosis of abscess. A formal incision and drainage was done. Pus culture showed staphylococcus aureus sensitive to Ampicillin, Cloxacillin, Gentamycin, Ciprofloxacin, Erythromycin and Cefotaxim. The wound was closed by secondary suturing some days later. The patient was advised to undergo definitive surgery at a later date. Even though bacterial thyroiditis is a known complication following FNAC, thyroid abscess has not been reported so far. Copyright 2002 - Indian Journal of Surgery.
