This
is a comprehensive book, forwarded by Alan DeCherney. The book consists of 19
chapters, which are written by thirty contributors (gynecologists and
interventional radiologists) who are pioneers in their field and who have many
years of experience in the areas they describe.
The
book discusses the epidemiology of fibroids and the different factors
correlated with the etiology. Histopathology of leiomyomata was discussed,
particularly the atypical leiomyoma, the leiomyosarcoma and the effect of
gonadotrophin releasing hormone agonist and embolization on the myoma.
Different
imaging procedures for evaluation of the fibroids, including the ordinary
x-ray, the ultrasound examination and the computed tomography and the MRI. The
value of each of these procedures was evaluated. Particular stress was done on
the imaging of the myoma pre-and post-uterine artery embolization.
Laparoscopic
management of uterine myoma was discussed in details in chapter 5 with emphasis
on advantages and disadvantages of laparoscopic myomectomy, the preoperative
use of gonadotropin-releasing hormone analogs and postoperative complications.
The
role of hysteroscopy in myomectomy was detailed and the technique of
hysteroscopic myomectomy, its role and its complications was also mentioned.
The
effect of myomas on pregnancy, possibilities of pregnancy loss, different types
of degeneration which might occur during pregnancy and the management during
pregnancy was explained.
A
whole chapter was dedicated to the expectant and medical management of
fibroids, which has an important role in several situations. Hysterectomy for
the treatment of fibroids including laparoscopic assisted vaginal hysterectomy
was also discussed and lots of illustrations were mentioned.
A big
part of the book is dedicated to uterine artery embolization including vascular
anatomic considerations. The technique of embolization, possible side effects
and the complications. The management of pain during and after uterine
embolization and a chapter was also dedicated to the patient selection,
indications and contraindications of embolization. The outcome of embolization
and the results were also detailed. The reproductive function after uterine
artery embolization was also detailed.
Finally
a chapter about the future of fibroid embolotherapy from the radiological
perspective.
