Sir,

We thank Dr. Wiwanitkit for his interest in our article. [1],[2] In case of dilated ventricles as in patients with normal pressure hydrocephalus there would be a generalized or global cortical perfusion deficit, hypoperfusion affecting selective areas like parietal and temporal or frontal and temporal cortices would not be observed as is expected in AD or FTD. We agree that we should include larger number of subjects in each group to increase the statistical validity of the data. Regarding the cost effectiveness of the perfusion SPECT done with ECD, it is comparable to the cost of a head NCCT and cheaper than routine MRI of the brain. In fact in India ECD kits are available from BRIT (Mumbai) at very economical pricing and in government hospitals the study costs between Rs 500 to Rs 1500 (US$ 10-30) only.
