Sir,

The method described to protect the pedicle of flaps to cover the posterior area in a heel defect is by no means simple as it is an invasive method, requires extra instrumentation and Ilizarov rings, which may not always be available in a plastic surgery unit. I have for long been using a simple plaster of Paris cast mould of about 16 to 20 layers roofed over an anterior cast. This not only effectively immobilises the limb and ankle, it also keeps off any pressure from the flap and the pedicle. [Figure - 1] and [Figure - 2]
