Resumen

La infeccion por VIH-1/SIDA es una importante pandemia que afecta a toda la humanidad. Actualmente 21,8 millones de personas estan infectadas con el VIH y alrededor de 7 000 contraen la infeccion cada dia. Desde el inicio de la epidemia se han reportado mas de seis millones de casos de SIDA. A pesar de recientes avances en la terapeutica con drogas antivirales, es evidente que el desarrollo de una vacuna preventiva es la unica opcion para lograr el control de la pandemia. El objetivo del presente trabajo es abordar con mayor profundidad el estudio de una de las regiones mas fascinantes de la envoltura viral: el lazo V3. En V3 se localiza el dominio principal de neutralizacion del VIH-1 y a su vez coexisten epitopos para celulas T auxiliadoras y citotoxicas. Por estas caracteristicas excepcionales, esta region ha sido incluida en casi todos los candidatos vacunales que se evaluan actualmente. Se resumen los principales hallazgos en cuanto a la estructura y el polimorfismo de esta region. A pesar de su variabilidad, determinados parametros estructurales se conservan para permitir a la region V3 mediar importantes funciones en el proceso infectivo. Se esboza una hipotesis general acerca del papel de V3 en el proceso de penetracion viral, y se discute su influencia sobre el tropismo viral. Varios trabajos indican la existencia de reactividad cruzada entre peptidos de V3. Algunos anticuerpos monoclonales humanos contra V3 definen epitopos considerablemente conservados y han logrado neutralizar cepas primarias. Por ultimo se ofrece una panoramica de las diferentes estrategias vacunales basadas en inmunogenos de la region V3.

Abstract

More than six million of AIDS cases have been reported since the beginning of the HIV/AIDS pandemic. In addition, there were 21.8 millions HIV-1 infected persons by June 1996. In spite of recent improvements in the therapeutics, the development of a preventive vaccine is critical for the control of AIDS. The aim of this review is to present a profound view of one of the most attractive regions of the viral envelope: the V3 loop. In V3, the principal neutralizing domain of HIV-1 has been located, as well as epitopes for helper and cytotoxic T cells. Because of its exceptional features, it has been included in the vast majority of the vaccine candidates in evaluation. The main findings about the structure and polymorphism within this region are summarized. Despite its variability, certain structural parameters are conserved in order to permit the V3 loop to mediate important functions in the viral infection. A general hypothesis about the role of V3 loop in the events of viral entrance and tropism is advanced. Different studies demonstrate the existence of cross-reactivity among V3 peptides. Some human monoclonal antibodies recognize conserved epitopes in V3 from different clades and neutralize primary isolates. However, the relevance of these aspects for vaccine development is still uncertain. Finally, an overview of the main vaccine strategies involving V3 immunogens is presented.
